Great Question
While I am lucky enough to skip much of Winter now I do get out on the odd cold ride and the frozen bottle is certainly a heart breaker !
A few suggestions
1) Keep one bottle in your back pocket of jersey, under your coat and rotate every 20-30 min, this also helps keep your riding no handed and pocket retrieval skills honed.
2) Check out insulated Bottles, CamelBack is one of the popular ones I have heard great things about for both Hot and Cold Weather/fluid needs.
3) I have seen people use hydration packs under coats
4) Make frequent stops (ie. every 90min to fill up or get a new bottle ... for Triathaletes this 'Transition' Setup is actually quite beneficial)
5) Start with warm/Hot Water in your bottles for a few more minutes of fluidity ... If you keep one under your jacket it might even help keep you warm longer!
